As soon as we have received your application, we will schedule an intake with you. We will then ask you to send us a number of documents. You can read more about this below. There are costs associated with the intake interview. These costs are covered by your basic insurance, but often fall within the excess amount.

Prior to your appointment, you will be asked to provide certain details. These details are important for your treatment. Below you can read why this data is needed.
1. Referral letter from GP or specialist
To get the costs of care reimbursed, you need a valid referral letter from your GP or specialist. Health insurers are not willing to reimburse without a referral letter, in which case you will be considered a self-referrer. A referral letter, with name and signature of the referring physician, is valid for one year from the date of issue and is only valid if the date of the letter is before or on the date of the first appointment with us. We therefore strongly recommend that you arrange a referral letter with your GP or specialist. The GP or specialist can forward this referral letter via Zorgdomein or you can send us the referral letter together with the completed forms by e-mail. If we have not received a referral letter, we are obliged to mark you as a self-referrer. As a result, your health insurer will not reimburse you for the treatment. Consequently, you will receive the invoice yourself and have to pay it directly to us. If, in the meantime or after receipt of this invoice, you still present the referral letter, we will charge a fee for this. These costs amount to € 150. If you have not been under treatment for longer than 1 year or if you have had a baby, you must re-register for an intake interview and you will need a new referral letter from your GP.
2. Medical history questionnaire
Prior to the intake, you should fill in the digital case history questionnaire via the patient portal. You will receive a separate invitation via e-mail (please check your junk e-mail). At the first visit to our clinic (intake), this data is studied by the doctor and discussed with you. The completed anamnesis list goes directly into your patient file.
If the medical history list is not complete, the doctor is unable to prepare the interview with you. The data will then be discussed with you during the first appointment. As going through this information takes extra time, a second appointment will have to be scheduled for the remainder of the intake.
3. Informed Consent
You must return the signed “Informed consent Care Costs and Health Insurers” form. If this form is not in our possession prior to your intake, we cannot proceed with the intake.
4. Your medical file; where relevant
You can use the form “request for medical information”. Only if you have been treated elsewhere, you should fill in this form, sign it and send it to (or hand it in to) the relevant department of the hospital where you were treated. If you have received the details yourself, you can send them to us immediately. You can also ask the departmental employee to email the details to us: aanmelden@nijgeertgen.nl
Children
Children are welcome at our clinic but we realise that this can be confrontational for our patients. We would therefore like to ask you to consider this whenever possible.
